NEXO
A daily clinical case to diagnose from up to six clues, with feedback measured on the ICD-10 tree.
Every day: One fictional clinical case a day, the same for every player: up to six progressive clues, and you name the diagnosis as an ICD-10 code with feedback on how close each guess sits in the ICD tree. New puzzle: Midnight.

NEXO is a daily diagnosis game for doctors and medical students, made in Brazil and available in six languages. Each day everyone gets the same fictional patient; you reveal clues one at a time and search for the ICD-10 diagnosis, and every miss tells you whether you were in the right category, group or chapter.
About NEXO
NEXO works like a medical Wordle. Each day a new fictional clinical case is published, such as a 67-year-old man with crushing chest pain. The first clue is visible and up to five more can be revealed, each costing points. You search for a diagnosis by name or ICD-10 code and submit it.
The feedback is the distinctive part. Instead of a plain wrong answer, each guess is placed on the ICD-10 tree: exact code, same category, same group, same chapter or a different chapter. Guessing coronary artery disease for a heart attack tells you that you were close; guessing pneumonia tells you that you were far off. Fewer clues and fewer guesses give a better score, and the result can be shared with the case number and the specialty.
The daily case is the same for all players and refreshes at midnight. Beyond it, a Continents mode covers diseases by regional epidemiology and a Multimedia mode adds X-rays, ECGs, endoscopy images and heart and lung sounds; these can be unlocked with points earned in the daily game, while paid Pro and Max plans add more volume. There are 649 cases across 25 specialties in Portuguese, English, Spanish, French, German and Russian, a global leaderboard, streaks and an iPhone app.
NEXO was founded by Thiago Longo Moraes, oncologist Alexei Peter dos Santos and data scientist Rafael Guimarães Malanga. It suits medical students, doctors and other clinicians, and curious players who enjoy medical mysteries.
How to play NEXO
- Open today's case and read the first clue.
- Search for a diagnosis by name or ICD-10 code and submit it.
- Read the proximity feedback (exact ICD, same category, same group, same chapter or different chapter) and reveal another clue if you need it.
- Reach the exact diagnosis using as few clues and guesses as possible, then share your result.
Tips
- Use the chapter feedback first: a 'different chapter' result means you are in the wrong organ system.
- A 'same group' result means you are close; look at neighbouring codes within that group.
- Think about age, sex and onset in the first clue before revealing more.
- Play the Continents and Multimedia modes to practise rarer and image-based diagnoses.
